Supporting youth and young adults in successfully transitioning from pediatric to adult oriented care is at the further
end of the developmentally appropriate care provided by pediatricians and pediatric subspecialist. In order to
support you in this important work, we need your input in how to share various resources and tools that can help you
and your staff in working with your patients and their families in navigating this milestone.
