Blood Lead Level Screening is required for children enrolled in Georgia Medicaid at 12 and 24 months of age or for children who screen positive on a blood lead level risk assessment. Practices can document on going blood lead levels conducted in their office and upload these test results into either the Georgia Registry of Immunization Transactions and Services (GRITS) or State Electronic Notifiable Disease Surveillance System (SENDSS). This form can assist in tracking your in office testing and then use to enter your data in either platform.
